By State law, your child must have a minimum number of
REQUIRED doses of vaccine to attend school. Parents MUST provide records, at the time of enrollment, that indicate the shots are complete or are in process.
All students entering seventh grade are required to be vaccinated with one (1) dose of meningococcal vaccine prior to the start of the school year.
All students entering the 12th grade are required to be vaccinated with two (2) doses meningococcal vaccine prior to the start of the school year. Parents must provide proof of the meningococcal immunization to their child's middle or high school on or before the first day of school in August.
Ohio law also requires
all students entering the seventh grade to receive a
Tdap (tetanus, diphtheria and pertussis)
booster immunization before the start of the school year. Parents must present proof of the Tdap immunization to their child's middle school on or before the first day of school in August. Students can receive the booster shot from their primary care physician or at local clinics and Ronald McDonald Care Mobile locations.
Immunization requirements for
preschool-aged children include Hepatitis A, Influenza, Pneumococcal Disease, and Rotovirus. Please review the recommended immunizations on the chart/list below.
